Before the visit

  • Current medicine list, with dosages
  • Recent test results or reports, if relevant to the visit
  • ID and any insurance/health card needed at registration
  • A clear note of the specific concern, so it doesn't get lost in conversation

At the hospital

Confirm the appointment time and doctor in advance where possible — walk-in waits with an elderly patient are harder than they need to be if a slot can be booked ahead.

After the visit

Update the medicine list if anything changed, and note any follow-up appointment before it's forgotten.
